New Hampshire Department of Environmental Services Residuals Management Forms

The proper treatment of wastewater leaves behind residual materials. These residual materials must be managed and may be disposed of in a beneficial way. The further treatment and management of these materials is permitted by NHDES and is most successful when information is submitted to the department effectively and accurately. Therefore, the department has the following applications and reporting forms to aid in the authorization of permits associated with these residual materials.

Forms

Exceptional Quality (EQ) Septage Certification Application

Application to certify filtrate and/or solids derived from septage treatment meet environmental standards, allowing septage derived material to be land applied at agronomic rates.

Septage and Sludge Hauler Application

Permit request to manage the removal and transportation of sludge or septage over public roadways in the state of NH.

Septage Transfer/Storage Tank Permit Application

Permit request for a septage transfer/storage tank(s) where the septage is consolidated for later removal, with a cumulative capacity of more than 20,000 gallons, and/or are used by more than one hauler.

Septage Holding Tank Permit-by-Notification

Any single septage hauler who wishes to temporarily hold septage in a tank or tanks less than 20,000 gallons.

Septage Management Rule Waiver Request

NHDES considers waivers in situations where strict compliance with the Septage Management Rules Env-Wq 1600 is not necessary to protect human health, safety and the environment.

Sludge Management Rule Waiver or Equivalency Request

NHDES considers waivers in situations where strict compliance with the Sludge Management Rules Env-Wq 800 is not necessary to protect human health, safety and the environment or for research purposes.

Sludge or Septage Facility or Septage Innovative/Alternative Technology Permit Application

Application for independent treatment facilities to process, treat and store sludge and septage.

Sludge or Septage Land Application (Site) Permit Application

Request a permit to stockpile and land apply Sludge/Biosolids or Septage, for beneficial use.

Sludge Quality Certification (SQC) Application

Apply for a sludge quality certification and demonstrate that the materials meet strict standards that are safe to be land applied and include nutrients or organic material, to improve crop-land or forested land; or can be used for reclamation.

Sludge Quality Certification (SQC) Renewal and Modification Application

Renewal of a Sludge Quality Certification according to Env-Wq 809.04.

Transfer, Modification, or Renewal of a Sludge or Septage Permit

This form is used to transfer, modify, or renew a sludge or septage land application (site) or facility permit.
