Facilities that emit air pollutants may be required to submit to NHDES reports that demonstrate their compliance status with applicable statutes, rules and permit requirements; and submit permit deviation reports when they are not in compliance. The reporting templates below can be used by a facility to assist it in submitting required reports. The templates include the minimum data fields necessary for the facility to submit a complete and acceptable report. The compliance report cover sheet can be used with a single or multiple submittal of reports to identify the report contents and assist NHDES to accurately and quickly enter the report(s) in our system.
